We appreciate your business and want your pets' stay to be the best possible. Please note that we are a veterinary clinic first, meaning we do have sick patients visiting our clinic, which is why we ask that all our boarding pets are up to date on their vaccinations. Below is our boarding form. It may seem lengthy, but it is the best way to ensure we have all the information we need to give your pet the best care possible. Please don't hesitate to call if you have any questions. Thank you! **Please note: This form does NOT reserve a spot for your pet in our facilities. It ONLY relays the information about your pet's stay with us. Please call our office to set up a boarding reservation.**
  • Please let us know what time you are wishing to pick-up your pet so we can do our best to have his/her items ready to go. Please do not pick up earlier than 8:00 am.
  • Please list the phone number(s) that we could reach you in the event of an emergency or concern regarding your pet.
  • Please list a trusted person and their phone number that can make medical decisions in the event that you are unable to be reached.
  • Emergency contact phone number
  • Please note any conditions that we should be aware of during your pet's stay. (i.e.: chronic/on-going conditions, skin conditions or any new problems that have been noted at home).
    There will be fees incurred for physical exam and any diagnostics/treatments deemed necessary.
    Please NOTE: If a flea/tick is seen on your pet during their stay, we will apply a flea/tick product at owner's expense.
    We understand that some pets enjoy being together at home, but sometimes, everyone needs their own space. Please indicate whether your pets are best together or separated. If one pet is known to chow on the other pet's food, maybe separation for feeding would be best to ensure everyone has the opportunity to eat while here.
  • PLEASE list all medications and doses here. (i.e.: Methimazole 5mg tablets - 1/2 tablet every 12 hours. Last dose 6/27/14 evening)
  • *If choosing to use our diet during your pet's stay, there will be no additional charge. However, with either choice, please list instructions of how much and how often to feed.
    *If you select No, we will discuss this problem with you at the time of discharge. In the event of a serious medical condition, if we are unable to reach you or your emergency contact, your pet will be treated at the Veterinarians discretion until you can be reached.*
